Who we are
Georgina Farmer is private physiotherapist registered with the HCPC PH49614 and works as a sole trader under the name ‘Physio4kids’. My website address is: https://physio4kids.org.uk.
Comments
When visitors leave comments on the website, we collect the data shown in the comments form, and the visitor’s IP address and browser user agent string to help spam detection.
An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
Media
If you provide or upload or images to the website, you should avoid images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

How long we retain your data

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
For users that register on https://physio4kids.org.uk (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Physio4kids.org.uk Website administrators can also see and edit that information.
Practice information
For the purposes of the Data Protection Act 1998, the Data Controller in relation to any personal data you or others supply in connection with any treatment is Georgina Phyllis Farmer (PZ7839862).
- Data Used will be for provision and administration of patient care only.
- Personal Details may include family, educational, photographic and medical information.
- Sharing of Information will be with other professionals involved with the data subject, this meets the CSP’s professional standards.
- Written and Printed Records will be stored and transported in a secure way.
- Computer Stored Information will be password protected and with network security software.

What rights you have over your data

If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
Practice information
In general, records relating to children and young people are kept until the child / young person reaches their 25th birthday or 8 years after the last entry if longer.
Paper notes will be shredded, pulped or burnt and disposed of securely. There are companies that provide a shredding service and issue a certificate.
Electronic notes will to be removed from my computer or any additional electronic device. All notes/letters/pictures and reports also need to be removed from any external back-up hard-drives and remote back up services.
